1. 1Which specific product do you use, and is it my own tissue or a purchased donor product?
2. 2Is it being used as a 361 HCT/P, or under an FDA investigational new drug application?
3. 3Who performs the injection: the physician of record, or a PA or nurse practitioner?
4. 4What is the total cost, and what happens if a second treatment is needed?
5. 5What outcome data do you keep on your own patients for my specific condition?
